To submit your registration for this conference please download the following form, complete all of the information and mail the completed form with a payment to one of the organizers listed below. All portions of the registration form should be completed, including accommodations and excursions. Information about Accommodations and Excursions is provided at this site on the associated links.

Registration fee covers all program materials, refreshment breaks, the reception, and the Kok River excursion. Registrants are responsible for all other meals, except dinner provided by hotel, and lodging and costs of excursions.

1. Kok River in Chiang Rai. November 12. 1 day. No fee.
2. Other Kok River activities in Chiang Rai. Arranged by participants separately. Half or full day. See website for information; contact vendor directly for prices.
3. Package Excursion of Golden Triangle, Doi Tung Royal Development Project, and Chiang Rai Area. Arranged by participants separately. 2 days. 2000 THB or $65.60
4. Kwan Payao lake area. November 14. 1 day. 1000 THB or $32.80
5. Mekong River at Chiang kong. 2000 THB or $65.60
6. Pai River near Maehongson. 15-20 November. 7500 THB or $246.00
Non-Thailand Registrants
Registration fees must be paid in U.S. dollars as a Money Order (with USA Banks) or Postal Money Order (issued at Post Offices) made payable to Richard L. Mayden. No credit cards or wire transfers are accepted.

Pre-register by 10 October 2007 to avoid late charges. If payment is not received by 10 October 2007 the late registration fees apply. Walk-in registrations to the meeting will be assessed an additional fee and there is no guarantee that any presentations are possible or excursion tickets or additional abstract CDs will be available.
Travel, Visas, Passport, and Currency: You are responsible for all travel arrangements, including procurement of your own passport, visas, and acquisition of proper currency sufficient for your stay.
Passport: Your travel to Thailand will require a passport from your country of residence. This can take several weeks to obtain without fees for expedited processing. Thus, you will need to process this immediately.
Visas: Assistance in obtaining documentation forms is available at Thailand embassies and other sources, such as various websites like PassportsPlus. A visa may or may not be required for your entry into Thailand. Participants are responsible for obtaining proper information for visas. Should you require a formal letter of invitation for this meeting to acquire a visa please contact Dr. Chavalit Vidthayanon.
If you do need a letter of invitation you should contact Dr. Chavalit Vidthayanon immediately for a letter of invitation to the Symposium to accompany your visa application. This letter may need to accompany your visa appilication.
